Gather the records
Admission and discharge dates, diagnosis, and itemized bills help. The outline of coverage lists what the policy pays.
- A hospital indemnity plan pays a fixed amount per day or stay. It does not replace the medical claim.
- Keep copies. The supplemental carrier may ask for the same pages again.
Send the form
File with the supplemental carrier using their claim form. The household portal does not submit that claim.
